Why Do I Need This Evaluation?

Many states and insurers require presurgical psychological or neuropsychological clearance because research shows that mental health evaluations can positively impact both treatment planning and postsurgical recovery. Your evaluation serves as a supportive step in preparing for surgery. It helps your care team understand how to best support your mental and emotional well-being before and after the procedure.

What to Expect

Before your scheduled evaluation, you’ll receive an intake packet that includes questions about your medical history and relevant background. These documents help lay the foundation for your personalized evaluation.


During your session, Dr. Karen Gordon will:


  • Conduct a thoughtful interview to learn more about your experience and your upcoming procedure
  • Answer any questions you may have
  • Include a loved one if you’d like them to be part of the conversation
  • Administer specific questionnaires and, depending on your surgery, cognitive or neuropsychological tests


This is not a pass/fail assessment. The results are used only to inform your care and will be shared directly with your referring provider.

How Long Is the Evaluation?

Each consultation typically lasts 1–2 hours. The time may vary depending on your individual needs, the complexity of your medical history, and how much support or clarification is needed during the appointment. 

What to Bring

  • To make the most of your evaluation, please bring:
  • Your completed intake packet and consent forms
  • Any previous assessment records
  • Relevant medical and psychological documents
  • All materials are reviewed as part of creating a full and accurate report.

How to Prepare

There’s no need to study—just come as you are. To feel your best, consider the following:


  • Get a good night’s rest
  • Bring your eyeglasses or hearing aids, if needed
  • Psychological Evaluations We Provide

  • Pre-Surgical Spinal Cord Stimulator (SCS) Evaluation
  • Pre-Surgical Bariatric Surgery Evaluation
  • Adult ADHD evaluations 
  • Surrogate Psychological Assessment


These assessments are designed to help plan for medical success and emotional well-being throughout the treatment journey.

Fees and Payments

Evaluation fees depend on complexity and the level of assessment required


A detailed estimate will be provided after the initial consultation and before testing begins.

Cancellation Policy

If you miss your appointment without 24-hour notice, a $75 cancellation fee will be charged. This fee must be paid before rescheduling. Your time matters, and so does ours—this policy helps ensure appointment availability for all clients.

What Happens After the Evaluation?

Once your assessment is complete, you may be scheduled for a feedback session to review your results, conclusions, and recommended next steps—unless your referral (e.g., for surrogacy) does not require this.


Your report will include practical insights and professional recommendations designed to support your health and future care.
